Did you know that Florida law requires you to keep your driver license and/or identification card address current within 10 days after moving? It's easy; update it online and save the $6.25 convenience fee!
Or, you can mail your Change of Address request form to the Division of Driver Licenses.
If you need to visit an office to change the address on your Florida driver license, commercial license (without Hazmat endorsement) and/or identification card, you will be asked to provide documents that will establish your identity. The documents are broken down into 4 categories listed below.
Note: If you have already presented these documents after January 1st, 2010 to obtain a Real ID compliant license (indicated by a gold star after your license number), you may not need to present them again at your next visit.
- Identification for US Citizens (Bring One)
Note: Customers born in 1935 or prior may be exempt from this requirement.
- Current Original US Passport or Passport Card
- Original/Certified US Birth Certificate from state or government agency
- Original US Certificate of Naturalization
- Original Report of Birth Abroad issued by the Department of State
- Social Security Number (Bring One)
Note: Must be in your current full name. If you have recently changed your name, you must visit the Social Security Administration first and wait approximately 24 hours for your name to update before visiting our office.
- Original Social Security Card or Stub with name and full SSN
- W-2 Form or pay stub with name and full SSN
- SSA-1099 or Non-SSA-1099 with name and full SSN
- Letter from Social Security with name and full SSN
- Residential Address (Bring Two; Must be in your name)
The most common proofs are:
- Vehicle/boat registration
- Voter registration card
- Automobile insurance policy
- Utility bill
- Deed or mortgage

- Legal Name Change (If Applicable)
Compare the name on your current driver license or ID card to the name on your US Citizenship documentation. If your full name from Step #1 is different, you must provide legal name change documents that link the names together. Note: Customers born in 1935 or earlier may be exempt from this requirement.

If you elect to not complete an Application Form in advance, you will be required to complete one prior to being served.
If your license or identification card is expired 2 years or more or you have a Hazmat endorsement, you must visit the State Driver License Office to complete your change of address.
Please Note: If completed in person, a new photo will be taken at the time of issuance.
