GENERAL INFORMATION

In order for our office to change the name on your Florida driver license, commercial license (without Hazmat endorsement) and/or identification card, you must:

  • Submit proof of Social Security #. Proof can be an original Social Security card, tax return, official letter from the IRS, W-2 form, pay check, DD-214, school record, military identification, official letter from the Social Security Administration, or Medicare card (with an A at the end).
  • Have your current (or expired less than 2 years) driver license in your possession.  If lost/stolen, your digital image must be on file
  • Have the following required original or certified document(s) to prove the “name chain” and/or change of name(s):
    • Marriage certificate
    • Divorce decree (must specifically state former name was “restored”)
    • Court order (must specifically state new name)

If your license or identification card is expired 2 years or more or you have a Hazmat endorsement, you must visit the State Driver License Office to complete your renewal.

Please Note:  A new photo will be taken at the time of issuance.
