Has your Florida driver license or identification card been lost or stolen?  If so, you may be eligible to request a replacement online and save the $5.25 convenience fee!

If you need to visit an office to request a replacement of your Florida driver license, commercial license (without Hazmat endorsement) and/or identification card:

• Submit proof of Social Security #. Proof can be an original Social Security card, tax return, official letter from the IRS, W-2 form, pay check, DD-214, school record, military identification, official letter from the Social Security Administration, or Medicare card (with an A at the end).
• Your digital image must be on file.

If your license or identification card is expired 2 years or more or you have a Hazmat endorsement, you must visit the State Driver License Office to complete your replacement.

Please Note:  If completed in person, a new photo will be taken at the time of issuance.
