The list below consists of the driver license services we offer. Some services require a vision exam to be administered. (Note: A new photo will be taken at the time of issuance).

  • Renewal of an expired (less than 2 years) Florida license/ID card, provided the person has not received any citations (speeding, careless driving, etc.) since his last renewal.  The person must have in his possession his license/ID card or if lost, the digital image must be on file.
  • Transfer of an out-of-state license to Florida, provided the person has not received any citations (speeding, careless driving, etc.) since his last renewal.  The person must have in his possession his out-of-state license, social security number (if issued) and a second form of identification, which can include a social security card.  NOTE: Must provide original or certified copy of US birth certificate or current US Passport if out-of-state license is from one of the following states:  Alaska, Connecticut, Hawaii, Illinois, Indiana, Iowa, Michigan, Minnesota, Nebraska, New Jersey, North Carolina, Oregon, Rhode Island, Tennessee, Texas, Utah, Vermont, Virginia, Washington or Wisconsin.
  • Transfer of an out of state CDL license to Florida, provided the person has not received any citations (speeding, careless driving, etc.) since his last renewal.  The person must have in his possession his out-of-state CDL license and social security card.  NOTE: Must provide original or certified copy of US birth certificate or current US Passport if out-of-state license is from one of the following states:  Alaska, Connecticut, Hawaii, Illinois, Indiana, Iowa, Michigan, Minnesota, Nebraska, New Jersey, North Carolina, Oregon, Rhode Island, Tennessee, Texas, Utah, Vermont, Virginia, Washington or Wisconsin. An original CDL with HAZMAT or School Bus endorsements must be processed at a full-service driver license office.  For HAZMAT endorsements, this is due to Federal requirements for finger printing, a background check, and legal presence verification (applicants must be a US Immigrant or a US Citizen).  For School Bus endorsements, this is due to the possibility of additional required testing.
  • Issuance of a Florida Only license, provided the person has not received any citations (speeding, careless driving, etc.) since his last renewal. The person must have in his possession his out-of-state license, social security number (if issued) and a second form of identification, which can include a social security card.  NOTE: Must provide original or certified copy of US birth certificate or current US Passport if out-of-state license is from one of the following states:  Alaska, Connecticut, Hawaii, Illinois, Indiana, Iowa, Michigan, Minnesota, Nebraska, New Jersey, North Carolina, Oregon, Rhode Island, Tennessee, Texas, Utah, Vermont, Virginia, Washington or Wisconsin.  A Florida Only license cannot be issued to persons with out-of-state CDL licenses.
  • Address Change on a valid Florida license/ID card, provided the person has in his possession his DL or if lost, the digital image must be on file.
  • Renewal/Address Change on a CDL license with no HAZMAT.
  • Lost/Stolen Florida DL/ID card, provided the digital image is on file.
  • D6 Clearances for:  (1) Failed to pay traffic fine, (2) Failed to appear for a traffic summons, (3) Failed to complete court ordered school, (4) School election when not eligible and (5) School election made-failed to attend.
  • Issuance of a Class E license from a Learner license, provided the person is at least 16 years old and has held his learner license for at least one year or until his 18th birthday (conviction free).  The person must have the following in his possession: (1) original Learner license or if lost, the digital image must be on file, (2) social security number (if issued), (3) second form of identification, which may be a social security card, (4) original DELAP waiver and (5) form HSMV 71022B (if parent or guardian is not present).
  • Issuance of an original ID Card, provided the person has in his possession his out-of-state license/ID card and can provide a second form of identification, which may be a social security card. NOTE: Must provide original or certified copy of US birth certificate or current US Passport if out-of-state license is from one of the following states:  Alaska, Connecticut, Hawaii, Illinois, Indiana, Iowa, Michigan, Minnesota, Nebraska, New Jersey, North Carolina, Oregon, Rhode Island, Tennessee, Texas, Utah, Vermont, Virginia, Washington or Wisconsin.
  • Name change on Florida license/ID card, provided the person has the following in his possession: (1) original license or if lost, the digital image must be on file and (2) original or certified copy (certified at the Clerk of Court) of name change documentation*, such as marriage license or court order.  (*The documentation will be returned to the customer.)
  • Addition of a Motorcycle Endorsement to a Florida license (class E or higher), provided the person has the following in his possession: (1) original DL or if lost, the digital image must be on file and (2) original certificate of completion from an approved Motorcycle Safety Course, Police Management Training Course or Motor Rider Course conducted by any branch of the military service.
  • Transfer an out-of-country license to Florida from any US territory, Canada or France, provided the person has not received any citations (speeding, careless driving, etc.) since his last renewal.  The person must have in his possession his original out-of-country license, original or certified copy of US birth certificate or current US passport, social security number (if issued) and a second form of identification, which can include a social security card.
